Delegate Designation Form

Use this form to list delegates and alternates for your association or unit. The recommended submission date for delegate names via this form is July 1, 2025. Submissions and changes will be accepted up to the time of the convention. Delegates must be NAP members and present at the Business Meeting at the Biennial Convention in Phoenix, AZ, to be eligible to represent their association or unit.
